Getting Started
You can fill your paperwork out ahead of time or arrive 10 minutes early to complete these forms. Please wear comfortable clothing so that you can move easily during the physical examination.
You’ll meet with Dr. Plante to discuss your medical history and have an examination. Following that, you’ll receive a report of his findings. You’ll have a diagnosis, a proposed care plan and go over informed consent. If you agree to begin, you’ll receive your first session.
Your initial assessment lasts 45-60 minutes. The future visits will be shorter.
Choosing to Stay Healthy
We’re not salespeople and will never pressure you into long-term care. Many of our patients choose to have maintenance care about once a month because they feel it benefits them. If you don’t feel it’s right for you, you can discontinue care at any time.
